dc.description.abstractLas malformaciones o defectos congénitos suelen producirse durante la vida intrauterina. Estas pueden ser fácilmente identificables (1–3) o pasar desapercibidas, y por lo tanto requerir el uso de paraclínicos específicos para detectar su presencia (2). Es por ello que las condiciones de salud materna durante la gestación y en torno al nacimiento son trascendentales para el curso de la vida de los niños afectados (3). Describir las manifestaciones neurológicas secundarias a infección congénita por Citomegalovirus (CMV) en pacientes menores de 5 años. Revisión de la literatura tipo Scoping review. Se consultaron las bases de datos PUBMED, EMBASE, ELSEVIER, SPRINGER, OVID, para el periodo comprendido entre 2010 y 2024, artículos que evaluaran las manifestaciones neurológicas secundarias a la infección congénita por Citomegalovirus (CMV). La revisión de la literatura muestra que la infección congénita por citomegalovirus (cCMV) tiene una prevalencia del 0.67% en recién nacidos, siendo tres veces mayor en países de ingresos bajos y medianos. Las manifestaciones neurológicas, que incluyen microcefalia, convulsiones y déficits motores, afectan al 65% de los pacientes con cCMV. Además, la hipoacusia neurosensorial se presenta en un 8% a 32% de los casos, con una prevalencia más alta en recién nacidos sintomáticos. La relación entre el momento de la infección durante el embarazo y la gravedad de las manifestaciones neurológicas resalta la importancia de la detección y prevención tempranas. Las imágenes cerebrales revelan hallazgos críticos como calcificaciones intracraneales y ventriculomegalia, indicadores de posibles secuelas neurológicas. Los biomarcadores plasmáticos como FLT4 y PPIA emergen como herramientas prometedoras para el diagnóstico de complicaciones neurológicas. El tamizaje neonatal y la intervención temprana son esenciales para mejorar los resultados en niños afectados por cCMV.
dc.description.abstractenglishCongenital malformations or defects usually occur during intrauterine life. These can be easily identifiable (1-3) or go unnoticed, and therefore require the use of specific paraclinicals to detect their presence (2). That is why maternal health conditions during gestation and around birth are transcendental for the life course of affected children (3). Objective. To describe the neurological manifestations secondary to congenital Cytomegalovirus (CMV) infection in patients under 5 years of age. Materials and methods. Scoping review. PUBMED/MEDLINE, LILACS, COCHRANE library, Scopus, EMBASE and Google Scholar databases were consulted for the period between 2010-2024, articles evaluating neurological manifestations secondary to congenital Cytomegalovirus (CMV) infection. A review of the literature shows that congenital cytomegalovirus (cCMV) infection has a prevalence of 0.67% in newborns, being three times higher in low- and middle-income countries. Neurological manifestations, including microcephaly, seizures, and motor deficits, affect 65% of patients with cCMV. Additionally, sensorineural hearing loss occurs in 8% to 32% of cases, with a higher prevalence in symptomatic newborns. The relationship between the timing of infection during pregnancy and the severity of neurological manifestations highlights the importance of early detection and prevention. Brain imaging reveals critical findings such as intracranial calcifications and ventriculomegaly, indicators of potential neurological sequelae. Plasma biomarkers like FLT4 and PPIA emerge as promising tools for diagnosing neurological complications. Education, neonatal screening, and early intervention are essential to improve outcomes in children affected by cCMV.
