Relación entre los niveles de lactoferrina en madres y recién nacidos de una clínica privada de Bogotá

La lactoferrina es una glicoproteína de la leche materna, con propiedades inmunomoduladoras, antioxidantes y antimicrobianas (2), planteándose valores diferentes según el kit que realice la medición, muestra empleada y población estudio. Mediante un estudio observacional longitudinal con muestreo no probabilístico por conveniencia, se realiza la medición de estos niveles en heces del recién nacido y leche materna en los tres momentos madurativos de la leche en una institución de salud de tercer nivel de Bogotá entre de 1 septiembre 2016 a 30 de noviembre 2016, siguiendo las instrucciones del fabricante de transporte, conservación y procesamiento del Elisa kit HK 329 Human Lactoferrin; para finalmente, determinar la relación entre estos resultados. Con el objetivo de lograr una muestra homogénea, se trata de mujeres entre 18 y 35 años de edad en puerperio y sus recién nacidos. Ingresan 65 binomios, pero solo se analizan 27 que completan el proceso de las 6 muestras requeridas (38 madres se retiran). En leche materna los niveles más altos están calostro, con una mediana de 1.125.177 ng/ml,; van disminuyendo los niveles progresivamente, encontrando en leche madura una mediana de 379.235 ng/ml. En heces de bebés, los niveles en primera muestra se registran entre 892 – 2754 ng/ml con mediana de 1946 ng/ml, en segunda muestra un rango intercuartílico de 1851 a 5801 ng/ml, aumentando en la tercera muestra con mediana de 4934 ng/ml. Finalmente, una relación inversa entre los niveles de lactoferrina en leche materna y heces de recién nacido, comparable a lo observado en la literatura.

Abstract
Lactoferrin is a maternal milk glycoprotein with immune-modulatory, anti-oxidant and antimicrobial properties (2), with different values according to the kit that performs the measurement, sample used and the population study. A longitudinal observational study with a non-probabilistic sampling for convenience is carried out to measure these levels in the newborn´s feces and breast milk in the three maturation moments of the milk in a third level Health Institution at Bogotá between from September 1st, 2016 to November 30th, 2016, following the instructions of the manufacturing company of transportation, preservation and processing of the Elisa HK 329 Human Lactoferrin Kit; so finally establish the relationship among these results. With the goal of getting a homogeneous sample, those are women between 18 and 35 years old in post-birth stage and their newborns. 65 binomials enter, but only 27 are analyzed that complete the process of the 6 required samples (38 mothers retired). In breast milk, the highest levels are colostrum, with a median of 1,125,177 ng / ml; gradually decreasing levels, finding in mature milk a median of 379,235 ng / ml. In feces of infants, the first sample levels were recorded at 892-2754 ng / mL with a median of 1946 ng / mL. The second sample shows an interquartile range from 1851 to 5801 ng / mL, increasing in the third sample with a median of 4934 Ng / ml. Finally, an inverse relationship between the levels of lactoferrin in breast milk and newborn feces, comparable to it was observed on the literature.
