dc.description.abstractEste caso clínico describe a un paciente varón de 23 años que presentó quilotórax derecho como manifestación inicial de un brote grave de lupus eritematoso sistémico (LES) y síndrome de Evans secundario. El quilotórax y la ascitis quilosa son características poco frecuentes del LES que pueden producirse debido a la acumulación de líquido rico en triglicéridos en cavidades serosas. Sin embargo, nunca se han descrito como manifestación inicial de un brote lúpico. El síndrome de Evans es una enfermedad rara caracterizada por anemia hemolítica autoinmune y trombocitopenia inmune, que puede ser secundaria al LES. Nunca se ha descrito la aparición concomitante de quilotórax y síndrome de Evans en el contexto del lupus eritematoso sistémico, y los mecanismos causales exactos de ambas entidades aún no se conocen por completo. En este informe, discutimos nuestro enfoque de este difícil caso para ampliar la comprensión de las manifestaciones clínicas del lupus eritematoso sistémico. Nuestros hallazgos subrayan la importancia de tener en cuenta las características poco frecuentes del lupus eritematoso sistémico y las enfermedades secundarias al evaluar a los pacientes con la enfermedad. dc.description.abstractenglishThis case report describes a 23-year-old male patient who presented with right chylothorax as the initial manifestation of a severe flare of systemic lupus erythematosus (SLE) and secondary Evans syndrome. Chylothorax and chylous ascites are rare features of SLE that can occur due to the accumulation of triglyceride-rich fluid in serous cavities. However, they have never been reported as the initial manifestation of a lupus flare. Evans syndrome is a rare disease characterized by autoimmune hemolytic anemia and immune thrombocytopenia, which can be secondary to SLE. The concomitant occurrence of both chylothorax and Evans syndrome in the setting of systemic lupus erythematosus has never been described, and the exact causative mechanisms of both entities are yet to be fully understood. In this report, we discuss our approach to this challenging case to broaden the understanding of the clinical manifestations of systemic lupus erythematosus. Our findings emphasize the importance of considering rare features of systemic lupus erythematosus and secondary diseases when evaluating patients with the disease. © The Author(s) 2023.eng
