Conciliación farmacoterapéutica al egreso hospitalario de adultos mayores: Una revisión panorámica

dc.description.abstractDentro de la temática a llevar en el siguiente trabajo, se entiende que la determinación de errores de medicación se puede mitigar y reducir de manera efectiva por medio de un procedimiento que se conoce como Conciliación Farmacoterapéutica, el cual consiste en la formación y ejecución de actividades que permitan revelar el perfil farmacoterapéutico (control medicamentoso de acuerdo a la necesidad actual del paciente), conllevando el registro pertinente de terapias que el paciente esté llevando a cabo y otro tipo de datos que puedan aportar información sobre las mismas (enfermedades actuales, riesgos del paciente, perfil hereditario, etc.). Sin embargo, en este tipo de conciliaciones farmacoterapéuticas, no se tienen en cuenta las variables que genera la creación de una administración medicamentosa adicional en la instancia hospitalaria a la que se somete al paciente, específicamente al egreso, sobretodo en poblaciones altamente expuestas a la polimedicación y agudización de efectos adversos como lo son los adultos mayores, asociado a cambios farmacocinéticos y farmacodinámicos relativos a su edad, además del uso que se dará a estos medicamentos al egreso hospitalario. La misión de este trabajo se centrará en la realización de una revisión panorámica para determinar el alcance que puede llegar a tener la ejecución de la conciliación farmacoterapéutica al egreso por medio de diferentes herramientas para identificar y corregir discrepancias de medicamentos y la correlación que puedan tener respecto la generación, agudización y relación directa de efectos adversos o daño causado por medicamentos, luego de un periodo de hospitalización (egreso). La revisión se enfocará en la población del adulto mayor y en las dificultades generadas en el seguimiento post-egreso hospitalario de este grupo, contando así los tipos de fallos relativos a la farmacovigilancia, fallos en la educación farmacoterapéutica al paciente y se estudiará la relación de efectos adversos o fallas medicamentosas relacionada directamente con los egresos hospitalarios.
dc.description.abstractenglishIt is understood that the determination of medication errors can be effectively mitigated and reduced by means of a procedure known as Pharmacotherapeutic Reconciliation, which consists of the formation and execution of activities that allow revealing the pharmacotherapeutic profile, involving the relevant record of therapies that the patient is undergoing and other types of data that can provide information about them. However, in this type of pharmacotherapeutic conciliations, the variables generated by the creation of an additional drug administration in the hospital instance to which the patient is submitted, specifically at discharge, are not taken into account, especially in populations highly exposed to polymedication and aggravation of adverse effects such as the elderly, associated to pharmacokinetic and pharmacodynamic changes related to their age, in addition to the use that will be given to these drugs at discharge from the hospital. The mission of this work will focus on the realization of a panoramic review to determine the scope of the execution of pharmacotherapeutic reconciliation at discharge by means of different tools to identify and correct drug discrepancies and the correlation they may have regarding the generation, aggravation and direct relationship of adverse effects or damage caused by drugs, after a period of hospitalization (discharge).
