Complicaciones quimioterapéuticas en pacientes adultos colombianos con leucemia linfoblástica aguda en el Hospital San Rafael: estudio descriptivo

dc.description.abstractLa leucemia linfoblástica aguda (LLA) es una neoplasia hematolinfoide con pronóstico variable y múltiples complicaciones; el pronóstico varía de acuerdo a múltiples factores, uno de estos está relacionado con el tratamiento quimioterapéutico. Describir las complicaciones presentadas en pacientes diagnosticados con leucemia linfoblástica aguda que recibieron tratamiento quimioterapéutico en el Hospital San Rafael. Se realizó un estudio retrospectivo de pacientes con leucemia linfoblástica aguda (LLA), teniendo en cuenta variables clínicas, paraclínicas y demográficas durante los años 2016 y 2017. Se incluyeron 116 pacientes con neoplasias hematológicas, 24 (20,6%) cursaron con LLA. Siendo en su mayoría LLA de tipo precursores de células B (79.2%). 86.4% pacientes estaban en fase de inducción a la quimioterapia, 86.5% presentaron neutropenia febril, 37.5% desarrollaron síndrome de lisis tumoral, 58.3% presentaron complicaciones infecciosas, y 45.8% fallecieron. Las infecciones de torrente sanguíneo y de vías respiratorias fueron los focos infecciosos más comunes por Escherichia coli y Klebsiella pneumoniae como agentes causales. Adicionalmente, la complicación más frecuente fue la neutropenia febril. La primera causa de muerte fue atribuida a infecciones complicadas por sepsis, sin embargo, sólo en un tercio de los pacientes se obtuvo aislamiento microbiológico.spa
dc.description.abstractenglishAcute Lymphoblastic Leukemia (ALL) is a neoplasm with variable prognosis and multiple complications; prognosis varies according to multiple factors, one of them are related with chemotherapeutic treatment. To describe the complications presented in patients diagnosed with Acute Lymphoblastic Leukemia that received chemotherapy treatment in San Rafael Hospital. A retrospective analysis was made from clinical records of adult patients diagnosed with Acute Lymphoblastic Leukemia admitted to hospitalization with some of their demographic variables and clinical outcomes described between the years of 2016 and 2017. 116 patients were included having hematological malignancies, 24 (20.6%) had Acute Lymphoblastic The majority had B cells precursor ALL (79.2%), 86.4% were in the chemotherapy induction phase, 86.5% developed febrile neutropenia, 37.5% had tumor lysis syndrome, 58.3% developed infectious complications, and 45.8% died. Bloodborne and respiratory tract infections were the most common sources of infection contributed by Escherichia coli and Klebsiella pneumoniae as causative agents. Also, the most frequent complication was febrile neutropenia. The first cause of death were infections complicated by sepsis, however, only one-third of the patients had a microbial isolation.eng
