This research analyzes the transformation needs in the IT department, in this case in Famisanar EPS, which is a health promoting entity that is dedicated to managing the health risk of its members through the development of agile and accessible processes. that allow the internal and external user to manage their needs safely and effectively, during this investigation a gap analysis is carried out identifying what the organization requires at the transformation level, the company has strategic alignment failures of the projects that are executed and there is a lack of knowledge at a vertical level of the precedents, this proposal proposes the dissemination of strategic planning from general management to the IT department and all its areas, together with the approach of an organizational transformation model that will apply transversally in the department. This consultative investigation, through an organizational diagnosis, the mission, vision and definition of value that Famisanar EPS has compared to the benefits sought within the IT department, this allows us to know the strategic planning of the organization and the capabilities with which that counts, and this is how the inclusion of new capabilities materialized in the results of the projects that are planned to be deployed through an organizational transformation model is defined, which is the product of this research. To do this, an execution plan is defined, which will be called a transition plan, which covers all the programs and projects that have been defined to eradicate the gaps identified during the analysis and diagnosis stage of the current processes within the IT department of the Famisanar EPS organization.
