Ivermectina subcutánea en el tratamiento de un síndrome de hiperinfección por Strongyloides stercoralis

dc.description.abstract | La estrongiloidiasis es una enfermedad causada por el nematodo Strongyloides stercoralis que es endémico en regiones rurales en países tropicales y subtropicales. Los pacientes inmunosuprimidos tienen un mayor riesgo de infección por este agente parasitario y pueden terminar en un síndrome de hiperinfección el cual conlleva un alto riesgo de muerte. Sin embargo, en Colombia y en el mundo no existe presentación parenteral de la ivermectina para uso en humanos. Lo anterior es un problema en aquellos pacientes que puedan tener comprometida la absorción enteral como es el caso de los pacientes con obstrucciones intestinales. Presentamos a continuación el primer caso de síndrome de hiperinfección por Strongyloides stercoralis en Colombia tratado con ivermectina subcutánea con el fin de incentivar la realización de estudios de farmacocinética y farmacodinamia para poder establecer esta vía de administración, como una alternativa de manejo en aquellos pacientes con riesgo de fracaso terapéutico con la vía oral. | spa |
dc.description.abstractenglish | Strongyloidiasisis a disease caused by the nematode Strongyloides stercoralisthat is endemic in rural regions in tropical and subtropical countries. Immunosuppressed patients have an increased risk of infection by this parasitic and are at risk of developing a hyperinfection syndrome which carries a higher risk of death. However, in Colombia and the world there is no parenteral presentation of ivermectin for the use in humans, which is an important problem in patients who have compromised enteral absorption, as in the case of patients with intestinal obstructions. We present the first case of hyperinfection syndrome by Strongyloides stercoralisin Colombia treated with subcutaneous ivermectin in order to encourage pharmacokinetic and pharmacodynamic studies to establish this route of administration in the future as an alternative to those patients who have a high risk of therapeutic failure with the oral route. | eng |
