Comparación del estado clínico periodontal y niveles de anticuerpos contra P. gingivalis en individuos con riesgo de desarrollar artritis reumatoide

dc.description.abstractAntecedentes: Los estudios de asociación de AR han dirigido su atención hacia las fases preclínicas o asintomáticas en individuos sanos con alto riesgo de desarrollar AR. Datos previos demuestran que los familiares en primer grado de pacientes con AR (FPG) presentan una condición periodontal inflamatoria significativa que podría modular la severidad de la presentación clínica de la AR. Comparar la condición clínica periodontal y los niveles de anticuerpos contra Porphyromona gingivalis (Pg) en individuos FPG de pacientes con AR y controles sanos. Se evaluaron 100 individuos FPG y 200 individuos sanos como grupo control, pareados por edad y género. Se realizó valoración reumatológica y periodontal. Se midieron anticuerpos contra Pg y anticuerpos antipéptido citrulinado (anti-CCP). Se compararon los grupos aplicando pruebas de McNemar y de Signos de Wilcoxon y se realizaron modelos de regresión logística condicional para establecer asociaciones de EP y otros factores asociados al riesgo a AR en FPG. El 79% del grupo FPG presentó EP comparado con un 56% del grupo control (p=0.001). Siendo el 15% EP severa en el grupo FPG vs 9% en el grupo control (p=0.009). Se observó asociación significativa de la presencia de EP en el grupo FPG (OR: 3, 95% CI 1.89–7.29). Un 17% de obesidad en el grupo FPG comparado con un 7.5% del grupo control (p=0.008); observándose una asociación significativa para la presencia de obesidad en FPG (OR: 2.9, 95% CI .03-8.28). La presencia de anticuerpos anti-CCP fue del 7% en FPG vs 2.5% del grupo control, mostrando una asociación significativa en el análisis bivariado (p=0.038). Aunque en el análisis multivariado no logró soportar la asociación (OR: 2.4, 95% CI 0.7-8.32), los resultados parecen indicar una tendencia a su incremento en individuos FPG. Los anticuerpos anti-Pg (IgG1-IgG2) no mostraron diferencias significativas, entre los grupos. La presencia de EP, la obesidad y la presencia de anticuerpos anti-CCP pueden ser variables que condicionan el riesgo en individuos FPG.spa
dc.description.abstractenglishAssociation studies in rheumatoid arthritis (RA) have been focused in the pre-clinical phases of the disease in asymptomatic individuals with higher risk to develop RA. Previous data has shown that first degree relatives (FDR) of patients with RA have a periodontal condition that may modulate the severity and the clinical presentation of RA. To compare the periodontal condition and levels of antibody against Porphyromona gingivalis (Pg) between FDR individuals and healthy controls. In total, 100 FDR individuals and 200 healthy controls paired by age and gender were included. Rheumatologic and periodontal assessment was performed as well as antibodies anti-Pg and antibodies anti-citrullinated peptides (anti-CCP). The group-comparisons were analyzed using McNemar and Wilcoxon tests. A conditional logistic regression analyses was performed to establish associations between periodontitis and additional risk factors for RA in FDR. In the FDR-group 79% had periodontitis in comparison with control group 56% (p=0.001). Fifty percent of severe periodontitis was observed in FDR vs 9% in control group (p=0.009). A significant association was found in the FDR individuals regarding the presence of periodontitis (OR: 3, 95% CI 1.89–7.29). In the group of FDR 17% had obesity, compared to 7.5% of obesity in the control group. Additionally, there was association related to the presence of obesity in the FDR group (OR: 2.9, 95% CI .03-8.28). Anti –CCP antibodies was found in 7% in FDR group vs 2.5 % in control group; in the bivariate analysis a significant association (p=0.038) was found. Although the multivariate analysis did not support the association (OR: 2.4, 95% CI 0.7- 8.32), this results shows a trend towards the increase of anti-CCP antibodies in FDR group (OR: 2.4, 95% CI 0.7- 8.32). Regarding the antibodies anti Pg (IgG1-IgG2), no differences was found between the groups.
