dc.description.abstract | Introducción La principal causa de trasplante renal en la infancia son las anomalías congénitas del riñón y la vía urinaria (CAKUT), las cuales justifican el 40- 50% de los trasplantes renales en niños, las otras causas incluyen la glomerulonefritis, la glomeruloesclerosis y otras lesiones. Se han observado diferencias en los desenlaces clínicos y la supervivencia post trasplante según la etiología de la enfermedad renal. Objetivo: Comparar las características demográficas, clínicas, los desenlaces y la supervivencia post trasplante renal según la etiología CAKUT versus No CAKUT en pacientes pediátricos en la Fundación Cardioinfantil de Bogotá Colombia entre 2005 y 2015. Metodología: Cohorte retrospectiva. Resultados: Se incluyeron 83 pacientes, 32 (38,6%) CAKUT y 51 (61,4%) No CAKUT. Se observaron diferencias en los desenlaces con mayor compromiso en los pacientes No CAKUT en cuanto a la creatinina sérica (p=0,05), la tasa de filtración glomerular a los 3 y los 5 años de seguimiento (p=0.006), y, la función renal (p=0,017). La supervivencia global fue del 90.7%, 93.8% para los pacientes CAKUT y 88.4% para los pacientes no CAKUT; la media para el tiempo de supervivencia global fue de 11.4 años, mayor para los pacientes CAKUT con 11.6 años (P=0.32). Conclusión Los desenlaces clínicos, la supervivencia del injerto y del paciente y el comportamiento de la función renal postrasplante varía de acuerdo con la etiología de la Enfermedad renal terminal (ERT) en pacientes pediátricos, con mayor compromiso de la función renal a los 3 y 5 años en pacientes no CAKUT. dc.description.abstractenglish | Introduction: The main cause of kidney transplantation in childhood is congenital anomalies of the kidney and urinary tract (CAKUT), which account for 40-50% of kidney transplants in children, the other causes include glomerulonephritis, glomerulosclerosis and others injuries. Differences in clinical outcomes and post-transplant survival have been observed according to the etiology of kidney disease. Objective: To compare demographic and clinical characteristics, outcomes, and post-renal transplant survival according to CAKUT versus Non-CAKUT etiology in pediatric patients at the Fundación Cardioinfantil de Bogotá, Colombia between 2005 and 2015. Methodology: Retrospective cohort. Results: 83 patients were included, 32 (38.6%) CAKUT and 51 (61.4%) Non-CAKUT. Differences were observed in the most compromised outcomes in Non-CAKUT patients in terms of serum creatinine (p=0.05), glomerular filtration rate at 3 and 5 years of follow-up (p=0.006), and, kidney function (p=0.017). Overall survival was 90.7%, 93.8% for CAKUT patients and 88.4% for non-CAKUT patients; the median overall survival time was 11.4 years, longer for CAKUT patients at 11.6 years (P=0.32). Conclusion: In pediatric patients the clinical outcomes of graft and patient survival, and kidney function post kidney transplant is according to the etiology of ESRD, with greater compromise of renal function at 3 and 5 years in non-CAKUT patients.
