Utilidad del diagnóstico visual (TFI) y por cuantificación de luz fluorescente (QLF) frente a microscopía de luz polarizada (PLM) y microrradiografía (TMR) en fluorosis leve y moderada en dientes primarios

dc.description.abstractLa fluorosis dental es un defecto del desarrollo del esmalte relacionado con la ingesta crónica de altos niveles de fluoruro durante la formación dental. El diagnóstico se basa en hallazgos visuales, que en dentición primaria son menos evidentes que en permanentes; se realiza por medio de índices como el TFI (Thylstrup y Fejerskov), que ha sido correlacionado histológicamente con microscopía de luz polarizada (PLM). Actualmente, existen técnicas que pueden medir la fluorosis cuantitativamente, como la cuantificación por luz fluorescente (QLF) y la microrradiografía (TMR), que evalúan pérdida mineral. Hasta el momento no se conocen estudios con QLF en fluorosis de dientes primarios. Determinar la utilidad del diagnóstico visual TFI y por QLF frente a PLM y TMR, en fluorosis leve y moderada en dientes primarios. Se clasificaron dientes primarios extraídos sanos y con fluorosis dental (n=112) con el índice TFI en imágenes de estereomicroscopía; se valoró fluorosis dental con QLF en umbrales 5-15-30 por medio de análisis de la superficie y región de interés (ROI) sobre imágenes de fluorescencia y se obtuvo el valor de área (A), pérdida de fluorescencia (∆F) y ∆Q; se obtuvieron secciones (80-120µm) de ROI y, se determinó pérdida mineral (∆Z), profundidad de la lesión (LD) y capa subsuperficial (SS) con TMR. En PLM se categorizaron en sano, fluorosis leve (TFI 1-2) y moderada (TFI 3-4). Se evaluó concordancia entre TFI y PLM, correlación entre TMR y QLF con coeficiente de correlación de Spearman, correlación de PLM y TFI con QLF con Kendall Tau, análisis ROC de QLF frente a diagnóstico visual e histológico; se describieron las características de dientes primarios TFI 0-4 con cada prueba. Se presentó concordancia moderada entre TFI y PLM (kappa=0.46), correlaciones: -nula entre QLF y TMR, -moderada entre QLF-TFI (tau A=0.5, ΔF=0.58, ΔQ=0.62; p≥0.001) y -débil entre QLF y PLM (tau A=0.32, ΔF=0.37 ΔQ=0.34; p≥0.001). Se obtuvo una sensibilidad de 73.9% y especificidad de 72.5% para ΔQ frente a PLM y 75% de sensibilidad y 78% de especificidad para ΔQ frente a TFI visual para fluorosis leve.spa
dc.description.abstractenglishDental fluorosis is a developmental defect of the enamel associated to high levels of fluoride intake during dental formation. The diagnosis of fluorosis is based on visual findings that in primary teeth are milder than in permanent teeth. The TFI index (Thylstrup y Fejerskov) is used on its diagnosis, and has been correlated histologically with polarized light microscopy (PLM). Currently, there are techniques that allow for an objective measurement of fluorosis, such as QLF (Quantitative Light Fluorescence) and TMR (Transverse microradiography). Both techniques assess percentage of mineral loss. There have been no published studies reporting on the correlation among QLF and a gold standard (PLM or TMR) on fluorosis in primary teeth. To determine the validity of visual diagnosis and Quantitative Light-induced Fluorescence (QLF) versus polarized light microscopy (PLM) and microradiography (TMR) of mild and moderate fluorosis in primary teeth. Methods extracted sound and fluorotic mild and moderated primary teeth (n=112) were examined using TFI via stereomicroscopy. Fluorosis was assessed with QLF by surface analysis and region of interest (ROI) on fluorescence images; A, ∆F and ∆Q values were obtained. Teeth were cut into 80-120µm sections that included the ROI to assess with TMR (∆Z, LD and SS were collected) and PLM; sections were classified as sound, mild or moderated fluorosis. Statistical analysis: it was established concordance between TFI and PLM, Spearman's rank correlation coefficient was obtained for TMR and QLF, Kendall tau was obtained for correlation between QLF vs TFI and PLM; results of the operating characteristics analysis (ROC) of QLF were compared to visual and histological diagnosis. A description of the fluorotic primary teeth categories was done. Agreement between TFI-PLM was 83.2% (kappa=0.46), a correlation between QLF and TMR was not found. A moderate correlation between QLF and TFI (tau A=0.5, ΔF=0.58, ΔQ=0.62; p≥0.001) and a weak correlation with PLM (tau A=0.32, ΔF= 0.37 ΔQ=0.34; p≥0.001) were found. A 73.9% sensitivity and 72.5% specificity were obtained for ΔQ versus PLM and a 75% sensitivity and 78% specificity for ΔQ versus TFI for mild fluorosis diagnosis.eng
