dc.description.abstractPaciente: Mujer, 59 años Diagnóstico Final: Melanoma maligno • melanoma de mucosas Síntomas: Tos • hemoptisis • pérdida de peso Medicamentos: — Procedimiento Clínico: Escisión de ganglios linfáticos • bilobectomía pulmonar Especialidad: Oncología • Cirugía Objetivo: Antecedentes: Reporte de Caso: Conclusiones : Enfermedad rara El melanoma primario de pulmón es un tumor raro que representa el 0,01% de los tumores primarios de pulmón, con sólo 40 casos descritos en la literatura. Los melanomas mucosos son tumores con una presentación biológica y clínica diferente a la de los melanomas cutáneos; por lo tanto, el enfoque terapéutico también difiere. Las tasas de supervivencia de los pacientes con melanoma primario de pulmón son mucho más bajas que las de los pacientes con melanoma cutáneo, y no existen pautas de diagnóstico o tratamiento para esta entidad. La cirugía radical es el tratamiento de elección cuando la enfermedad es resecable. Se desconoce la eficacia de los tratamientos actuales establecidos para el melanoma cutáneo (p. ej., inmunoterapia y terapia dirigida) en este subgrupo en particular. Presentamos el caso de un paciente que consultó por tos y hemoptisis. La fibrobroncoscopia reveló una masa endobronquial y las imágenes de tomografía computarizada sugirieron una masa irresecable. El paciente fue diagnosticado inicialmente con un melanoma primario de pulmón irresecable con un estadio clínico IIIB (T4N2M0). Esta lesión logró una respuesta parcial tras el tratamiento con pembrolizumab, lo que permitió realizar una cirugía radical, consiguiendo una resección completa con márgenes negativos y una adecuada evolución postoperatoria. A pesar de los retrasos en nuestro sistema de atención médica, actualmente está viva y libre de enfermedades más de 24 meses después del diagnóstico. La inmunoterapia puede reducir el tamaño del melanoma mucoso hasta el punto de que puede ser resecable y este enfoque terapéutico aumenta las oportunidades de supervivencia de estos pacientes.spa

Mucosal melanomas are tumors with a biological and clinical presentation that differs from that of cutaneous melanomas; therefore, the therapeutic approach differs as well. Survival rates of patients with primary melanoma of the lung are much lower than those of patients with cutaneous mela-noma, and there are no diagnostic or treatment guidelines for this entity. Radical surgery is the treatment of choice when disease is resectable. The effectiveness of current established treatments for cutaneous melanoma (eg, immunotherapy and targeted therapy) is unknown in this particular subgroup. We present the case of a patient who presented with cough and hemoptysis. The fiberoptic bronchoscopy revealed an endobronchial mass and the computed tomography images suggested an unresectable mass. The patient was initially diagnosed with an unresectable primary lung melanoma with a clinical stage IIIB (T4N2M0). This lesion achieved partial response after treatment with Pembrolizumab, which allowed radical surgery to be performed, achieving complete resection with negative margins and adequate postoperative evolution. Despite the delays in our health care system, she is currently alive and disease-free more than 24 months after diagnosis. Immunotherapy can reduce the size of mucosal melanoma to the point that it can be resectable and this therapeutic approach increases the survival opportunities of these patients.eng
