Frecuencia del síndrome de realimentación en neonatos prematuros y/o con restricción de crecimiento intrauterino que recibieron nutrición parenteral en el Hospital Simón bolívar periodo 2021 a 2023

dc.description.abstractIntroducción: La nutrición parenteral es una estrategia nutricional para mejorar los procesos metabólicos de neonatos que lo requieran, en este caso recién nacidos pretérmino y/o con restricción de crecimiento intrauterino. En algunos casos, se puede presentar síndrome de realimentación que es definida como hipofosfatemia sistémica asociada o no a desequilibrio hidroelectrolítico. Este se asocia con aumento en la morbimortalidad a largo plazo y/o discapacidades neurológicas. Se pretendió calcular la frecuencia del síndrome de realimentación en esta población en la institución entre 2021-2023 y evaluar la ganancia de peso en ambos grupos. Métodos: Estudio observacional descriptivo con intención analítica, que incluye la totalidad de neonatos con bajo peso y/o RCIU que recibieron nutrición parenteral. Se analizan datos maternos, neonatales y la ganancia de datos antropométricos al igual que características de la nutrición parenteral por grupos, se presentan en términos descriptivos. Resultados: Entre 138 neonatos con bajo peso y/o RCIU, la frecuencia de síndrome de realimentación es del 21,0%. En el grupo de síndrome de realimentación, el peso y la talla al nacer fue menor, pero la ganancia de peso total fue significativamente mayor en este grupo (13,2g/día vs 7,6g/día). Si bien se presentaron algunos casos de hipofosfatemia asociado a otras alteraciones hidroelectrolíticas, lo que predominó fue la hipofosfatemia con valores séricos < 4.5 mg/dL. Discusión: Llama la atención que se ha reportado en la literatura una asociación entre el síndrome de realimentación y el inicio de aporte de proteínas elevado, sin embargo, en nuestro estudio no se encontró dicha causalidad. El presente estudio se presenta como punto de partida a nivel latinoamericano para futuras investigaciones.
dc.description.abstractenglishBackround: Parenteral nutrition is a nutritional strategy to improve the metabolic processes of neonates who require it, in this case, preterm newborns and/or those with intrauterine growth restriction. In some cases, refeeding syndrome can occur, which is defined as systemic hypophosphatemia, with or without associated hydroelectrolytic imbalance. This is associated with an increase in long-term morbidity and mortality and/or neurological disabilities. The aim was to calculate the frequency of refeeding syndrome in this population at the institution between 2021-2023, and to evaluate growth gain in both groups. Methods: A descriptive observational study with an analytical intention was performed, including all neonates with low birth weight and/or IUGR who received parenteral nutrition. Maternal and neonatal data, as well as anthropometric data gain and parenteral nutrition characteristics by groups, were analyzed and presented in descriptive terms. Results: Among 138 neonates with low birth weight and/or IUGR, the frequency of refeeding syndrome was 21.0%. In the refeeding syndrome group, birth weight and length were lower, but the total weight gain was significantly higher in this group (13.2 g/day vs. 7.6 g/day). Although some cases of hypophosphatemia associated with other hydroelectrolytic disorders were presented, hypophosphatemia with serum values < 4.5 mg/dL. Discussion: It is noteworthy that an association between refeeding syndrome and the initiation of high protein intake has been reported in the literature; however, no such causality was found in our study. This study serves as a starting point at a Latin American level for future research.
