Eventos morales y angustia moral en los profesionales de la salud que realizan la práctica de la interrupción voluntaria del embarazo en fetos viables: una revisión narrativa

dc.description.abstractLa despenalización de la Interrupción voluntaria del Embarazo (IVE) es un constructo histórico ligado a creencias morales inmersas en interpretaciones científicas, metafísicas y religiosas respecto al estatus moral del feto. El abordaje de los criterios de estatus moral independientes y dependientes del feto y su gradualidad dada por la potencialidad de este nos lleva a la construcción del concepto del feto como paciente que genera tensiones entre los derechos de los fetos y las personas embarazadas y se agudizan cuando se trata de viabilidad gestacional. La conciencia es una función que discierne verdades morales. La objeción de conciencia hace parte del conflicto legal vs. moral de la IVE ¿Y en aquellos casos donde no se objeta conciencia, pero si se realiza una práctica que genera culpa, remordimiento y vergüenza? La angustia moral desde Georgina Morley es la unión entre un acontecimiento moral y una experiencia de angustia psicológica dentro de una relación causal directa entre ellas identificada a través de cinco tipos de eventos morales: restricción, tensión, conflicto, dilema e incertidumbre. Se realizó una revisión de 13 artículos publicados entre los años 2000 a 2025 que identifican las cinco categorías de angustia moral propuestas en el marco de la práctica de la IVE. También se describen experiencias de satisfacción moral. Se concluye que es urgente implementar intervenciones educativas y emocionales que ayuden a los profesionales a manejar estos conflictos morales dentro de un modelo de responsabilidad profesional, fortaleciendo su bienestar psicosocial y su capacidad de atención ética y empática.
dc.description.abstractenglishThe decriminalization of Voluntary Interruption of Pregnancy (VIP) is a historical construct linked to moral beliefs embedded in scientific, metaphysical, and religious interpretations regarding the moral status of the fetus. The approach to independent and dependent criteria of moral status of the fetus, and its gradual nature based on its potentiality, leads to the construction of the concept of the fetus as a patient. This generates tensions between the rights of fetuses and pregnant individuals, which intensify in cases involving gestational viability. Conscience is a function that discerns moral truths. Conscientious objection is part of the legal versus moral conflict surrounding VIP. But what about cases where there is no conscientious objection, yet the practice results in feelings of guilt, remorse, and shame? Moral distress, as defined by Georgina Morley, is the intersection between a moral event and a psychological experience of distress, within a direct causal relationship identified through five types of moral events: constraint, tension, conflict, dilemma, and uncertainty. A review of 13 articles published between 2000 and 2025 was conducted, identifying these five categories of moral distress within the context of VIP practice. Experiences of moral satisfaction are also described. It is concluded that there is an urgent need to implement educational and emotional interventions to help professionals manage these moral conflicts within a model of professional responsibility, thereby strengthening their psychosocial well-being and their capacity for ethical and empathetic care.
