Update on delirium assessment in the Intensive Care Unit: A scoping review

| dc.description | Introduction: Delirium is a clinical syndrome affecting patients admitted to Intensive Care Units (ICUs). It is characterized by an acute and fluctuating disturbance of consciousness, along with reduced attention and changes in thinking and perception. Delirium is classified into three types: hyperactive, hypoactive, and mixed. It occurs predominantly in older adults admitted to the ICU. Objective: To identify and describe the scales used to diagnose and assess delirium in the ICU. Methods: A scoping review was conducted in PubMed, LILACS, and Elsevier databases between 2000 and 2023 using keywords such as "Delirium," "Intensive Care," "Scales," and "Risk Factors." Articles were evaluated using the RedCASPe tool, and those scoring over 75% in quality and validity were selected. Ten articles meeting the desired approach were selected. Results: Four instruments were identified for assessing delirium in intensive care units, with specificity ranging from 82.4% to 100%, sensitivity from 30% to 93%, and reliability indicated by Cronbach’s α values between 0.8 and 0.9. Each scale demonstrated adequate validity, feasibility, ease of implementation, assessment, and interpretability for clinical use. Conclusions: Current approaches to delirium in the ICU emphasize comprehensive and collaborative management, and early detection is achieved with instruments such as the CAM-ICU and the PRE-DELIRIC. Addressing risk factors, such as infections and medication use, through pharmacological and non-pharmacological strategies is crucial. Early mobilization and family involvement are essential. | en-US |
| dc.description | Introducción: El delírium es un síndrome clínico que afecta a los pacientes ingresados en las Unidades de Cuidados Intensivos (UCI). Se caracteriza por una alteración aguda y fluctuante de la conciencia, acompañada por disminución de la atención y cambios en el pensamiento y la percepción. El delírium está clasificado en tres tipos: hiperactivo, hipoactivo y mixto. Se presenta primordialmente en adultos mayores que ingresan en la UCI. Objetivo: Reconocer las escalas utilizadas para el diagnóstico y evaluación del delírium en la UCI. Métodos: Se realizó una revisión de alcance en Pubmed, LILACS y Elsevier entre 2003 y 2023, utilizando términos clave como "Delirium", "Intensive Care", "Scales" y "Risk Factors". Los artículos se evaluaron utilizando la herramienta RedCASPe y se seleccionaron los que demostraron una calidad y validez superiores al 75%. Se seleccionaron 10 artículos que cumplieron con el enfoque deseado. Resultados: Se identifican cuatro instrumentos para evaluar el delirio en unidades de cuidados intensivos, con especificidad del 82.4% al 100%, sensibilidad del 30% al 93% y confiabilidad (α de Cronbach 0.8-0.9). Cada escala presenta validez, aplicabilidad, implementación, evaluación e interpretabilidad para su uso clínico. Conclusiones: El enfoque actual del delírium en la UCI enfatiza un abordaje completo y colaborativo y la detección temprana se logra con instrumentos como el CAM-ICU y el PRE-DELIRIC. La identificación de estrategias preventivas, farmacológicas y no farmacológicas, son cruciales para tratar factores de riesgo como infecciones y medicamentos. Caracteriza-se por uma alteração aguda e flutuante da consciência, acompanhada por redução da atenção e mudanças no pensamento e na percepção. O delírio é classificado em três tipos: hiperativo, hipoativo e misto. Apresenta-se predominantemente em idosos internados na UTI. Objetivo: Reconhecer as escalas utilizadas para o diagnóstico e avaliação do delírio na UTI. Métodos: Foi realizada uma busca em bases de dados como PubMed, LILACS e Elsevier entre 2018 e 2023, utilizando termos-chave como "Delirium", "Intensive Care", "Scales" e "Risk Factors". Os artigos foram avaliados por meio da ferramenta RedCASPe, selecionando aqueles com qualidade e validade superiores a 75%. Foram incluídos 10 artigos que atenderam ao foco desejado. Resultados: Os 10 artigos selecionados foram analisados considerando critérios de inclusão como o ano de publicação (2018-2023), o idioma (inglês, espanhol e português), os fatores de risco e o uso de escalas para avaliação do delírio na UTI. Conclusões: A abordagem atual do delírio na UTI enfatiza uma estratégia abrangente e colaborativa. A detecção precoce é possível com instrumentos como o CAM-ICU e o PRE-DELIRIC. A identificação de estratégias preventivas, farmacológicas e não farmacológicas é crucial para manejar fatores de risco como infecções e o uso de medicamentos.
