Cambios en la presión intraocular (PIO) durante el embarazo: revisión sistemática de la literatura

dc.description.abstractObjetivo: Identificar los cambios en la presión intraocular durante los diferentes trimestres de embarazo. Metodología: La búsqueda sistemática se realizó en Medline (Pubmed), Embase y Web of Science. Se seleccionaron estudios observacionales en gestantes en los diferentes trimestres del embarazo y posterior al parto. Resultados: Fueron incluidos 25 estudios con seguimiento de la PIO en el primer, segundo, tercer trimestre del embarazo y posparto. En general el promedio de PIO en el primer trimestre fue de 15.5 mmHg, en el segundo trimestre de 13.4 mmHg y en el tercer trimestre 11.62 mmHg, Se reportó diferencia estadísticamente significativa en los diferentes trimestres de gestación en 13 estudios con un valor de p < 0.005. Todos los estudios coincidieron con un descenso de la PIO en promedio de 2 mmHg entre el primer y segundo trimestre y 1.78 mmHg entre el segundo y tercer trimestre. Respecto al posparto, 2 estudios indicaron un incremento hasta del 3 % de la PIO tres meses después del parto. Conclusiones: Se identificaron variaciones en la presión intraocular durante la gestación siendo, menor a medida que avanza el embarazo y mayor luego del parto. Se proponen futuros estudios de seguimiento después del parto para analizar si estas fluctuaciones pueden generar cambios que den lugar a una patología en el polo posterior.
dc.description.abstractenglishObjective: To identify changes in intraocular pressure during different trimesters of pregnancy. Methodology: The systematic search was performed in Medline (Pubmed), Embase and Web of Science. Observational studies in pregnant women in different trimesters of pregnancy were selected. Results: Twenty-six studies with IOP follow-up in the first, second, third trimester of pregnancy and postpartum were included. In general, the average IOP in the first trimester was 15.5 mmHg, in the second trimester it was 13.4 mmHg and in the third trimester 11.62 mmHg. A statistically significant difference in the different trimesters of pregnancy was reported in 13 studies with a value of p < 0.005. All studies agreed with an average IOP decrease of 2 mmHg between the first and second trimester and 1.78 mmHg between the second and third trimester. Regarding postpartum 2 studies indicated up to a 3 % increase in IOP 3 months after delivery. Conclusions: Variations in intraocular pressure were identified during gestation being lower as pregnancy progresses and higher after delivery. Future studies are proposed to extend the follow-up after delivery in order to analyze if these fluctuations can generate changes that could lead to pathology in the posterior pole.
