dc.description.abstractLa infección urinaria es una patología frecuente en la población pediátrica y es una de las principales causas de consulta en el servicio de urgencias, donde es indispensable confirmar de manera rápida la infección para una toma de decisiones adecuada y eficaz que permita dar un tratamiento oportuno y evite la presencia de secuelas a largo plazo1,2,3. El urocultivo es el método de confirmación de la patología pero su reporte solo se obtiene hasta 48 horas después de la siembra de la muestra1,4,5. El Uro 4-system es una técnica utilizada actualmente basada en la nefelometría laser para detectar la presencia de bacteriuria cuyos resultados se obtienen a las 4 horas de procesada la muestra. Objetivo: Evaluar las características operativas del Uro 4-system comparado con el patrón de oro que es el urocultivo en pacientes pediátricos con sospecha de infección de vías urinarias. Metodología: Es un estudio de evaluación de una prueba diagnóstica – características operativas, de una prueba de detección rápida de bacteriuria como es el Uro 4-system y se compara con una prueba de referencia que es el urocultivo. Resultados: Se recolectaron 100 muestras de orina de pacientes pediátricos, que fueron atendidos en los servicios ambulatorios y hospitalarios de la Fundación Cardioinfantil durante el 1 de septiembre y el 31 de octubre de 2010. Las características operativas y los intervalos de confianza del 95% del Uro 4-system para detectar infección de vías urinarias fueron: sensibilidad del 100% (87.1 – 100); especificidad del 98.6% (92,7 – 99,8%), valor predictivo positivo de 96,3% (81,7 – 99,3), valor predictivo negativo del 100% (95-100%). Conclusiones: Un Uro 4-system positivo prácticamente confirma el diagnóstico de infección de vías urinarias en la edad pediátrica, pero un resultado negativo no necesariamente, bajo los resultados del estudio, descarta la presencia de infección de vías urinarias.spa
dc.description.abstractenglishThe urinary tract infections is a frequent pathology in the pediatric population and is one of the main reasons for consultation on the emergency department where it„s essential to confirm the infection rapidly for a suitable and effective way to take decisions allowing to give an accurate treatment and avoid the presence of sequels at a long term1,2,3. The method to confirm the pathology is the urine culture but it‟s report is obtained only up to 48 hours after the sowing of the sample1,4,5. On the other hand, The Uro 4-system is a technique used nowadays to detect the presence of bacteriuria based on laser nefelometry whose results are obtained within 4 hours after processing the sample. Objective: Evaluate the operative characteristics of the Uro 4-system compared with the urine culture, the golden pattern on pediatric patients with suspicion of urinary tract infections. Methodology: It‟s the evaluation study of a diagnostic test – operative characteristics of a fast detection test of bacteriuria as it is the Uro 4- system, compared with a reference test which is the urine culture. Results: There were gathered 100 urine samples of pediatric patients who were attended in the ambulatory and hospital services of the Fundación Cardioinfantil between September 1 and October 31, 2010. Operative characteristics and 95% confidence intervals of the Uro 4 system to detect urinary tract infections were: 100% sensibility (87.1 - 100); 98.6 % specificity (92,7 - 99,8), 96,3 % predictive positive value (81,7 - 99,3), 100 % predictive negative value (95-100). Conclusions: A positive Uro 4-system practically confirms the diagnosis of urinary tract infections on pediatric age patients, but a negative result, not necessarily under the results of the study, rejects the presence of urinary tract infections.eng
