dc.description.abstract | La ventilación mecánica invasiva (VMI) es una de las terapias más comunes que se utilizan en las unidades de cuidados intensivos (UCI), su objetivo es suplir de forma parcial y transitoria la función del sistema respiratorio. Esta intervención tiene múltiples causas, riesgos y retos, siendo el síndrome de dificultad respiratoria aguda (SDRA) una condición que implica un reto teórico práctico mayor para la ventilación mecánica y que por lo mismo ha significado un avance de la misma (1). Sin embargo, las medidas que tienen un impacto significativo en los desenlaces son limitadas, entre estas se encuentra la presión positiva al final de la espiración (PEEP), pero no existe, un consenso acerca del mejor método para titular la PEEP en pacientes bajo VMI. Aunque la aparición en la última década de la tomografía por impedancia eléctrica (TIE) en la práctica clínica, introdujo un nuevo método para titular la PEEP y por lo tanto una nueva incertidumbre. | |
dc.description.abstractenglish | Invasive mechanical ventilation (IMV) is one of the most common therapies used in intensive care units (ICUs), aimed at partially and transiently replacing respiratory function. This intervention is associated with multiple causes, risks, and challenges, with acute respiratory distress syndrome (ARDS) posing a significant theoretical and practical challenge for mechanical ventilation and thus driving advancements in the field (1). However, interventions with a substantial impact on outcomes remain limited. Among these is positive end-expiratory pressure (PEEP); yet, consensus on the optimal method for titrating PEEP in IMV patients is lacking. The introduction of electrical impedance tomography (EIT) into clinical practice in the last decade has presented a new method for PEEP titration, thereby introducing new uncertainties. | |
