Asociación entre apnea del sueño y ataque cerebro vascular

Resumen

El síndrome de apnea hipopnea del sueño (SAHOS) es un trastorno que consiste en disrupción de la respiración normal durante los periodos de sueño, cuando la faringe colapsa, ocasionando obstrucción parcial o total de la vía aérea. Se ha asociado con múltiples patologías vasculares que afectan la función endotelial. Los Ataques cerebrovasculares (ACV) pueden alterar por múltiples mecanismos el correcto funcionamiento de la función ventilatoria pulmonar. Esta asociación no está completamente dilucidada y es por ese motivo que realizamos un estudio retrospectivo para determinar la existencia de una asociación de estas dos patologías en SOMNARUN. Encontramos que 22 de los 912 pacientes de la muestra presentan antecedentes de ACV. Los resultados demuestran que las enfermedades vasculares son más frecuentes en las personas con SAHOS con Índice de apnea hipopnea (IAH) mayor de 15 persistiendo cuando se delimita a hipertensión (HTA). No se encontró asociación significativa entre la presencia de SAHOS con IAH mayor de 15 y ACV. Se requieren nuevos estudios con mayor proporción de pacientes con ACV para tratar de dilucidar esta asociación.

Abstract

The Obstructive Sleep Apnea Hypopnea Syndrome (OSAS ) is a disorder that involves disruption of normal breathing during sleep periods, when the pharynx collapse, causing partial or complete obstruction of the airway. It has been associated with multiple vascular pathologies affecting endothelial function. Cerebrovascular attacks (Stroke) can affect the correct lung ventilatory function by many reasons. This association is not fully understood yet and this why we have conducted a retrospective study to determine the existence of an association between these two diseases in SOMNARUN . We found that 22 of the 912 patients of the sample have had a stroke. The results demonstrate that vascular diseases are more common in people with OSA (p < 0001) with apnea hypopnea index (AHI) higher than 15 specially when persisting hypertension (OR: 2.068 ) . However no significant association between the presence of OSAS was found with AHI higher than 15 and stroke (OR : 3.63 p 0.220 ) . Further studies with a greater proportion of patients with stroke are required to explain this association.
