Parámetros biométricos y peso fetal estimado de los fetos en una población local. Una comparación con estándares internacionales y curvas latinoamericanas

Resumen

Objetivo: Describir el peso fetal estimado (PFE) según la fórmula de Hadlock y las variables biométricas fetales: diámetro biparietal (DBP), circunferencia cefálica (CC), circunferencia abdominal (CA), longitud del fémurl (LF) en una muestra de embarazadas de una población local y comparar sus mediciones con las curvas y los percentiles internacionales publicados por el projecto INTERGROWTH-21st y Lagos et al., en Chile. Métodos: Se obtuvieron datos de informes de ultrasonido de embarazos únicos entre las semanas 16 y 39 de embarazadas en Bogotá-Colombia. La edad gestacional (EG) se confirmó por la longitud craneo caudal (LCC). Se evaluaron los parámetros BPD, CC, CA, LF y se calculó el PFE de acuerdo con la fórmula de Hadlock. Se realizó un análisis descriptivo de cada parámetro biométrico y se calculó la media, la mediana y la desviación estándar por semana. Luego, estos datos se compararon con las curvas y los percentiles del proyecto INTERGROWTH-21st y las curvas latinoamericanas, usando la prueba t de Student y la prueba de rango con signo de Wilcoxon. Resultados: se analizaron un total de 1133 informes ecográficos de 664 fetos, y los datos se presentaron en gráficos con la representación de los percentiles 3, 50 y 97 del proyecto INTERGROWTH-21st y los percentiles 10, 50 y 90 de Lagos et al.. La CA y la LF fueron los parámetros que mostraron más diferencias estadísticamente significativas con las curvas internacionales. Conclusiones: Las curvas de referencia del proyecto INTERGROWTH-21st y Lagos et al., muestran diferencias cuando se compara el crecimiento fetal de esta población. Los gráficos personalizados o locales pueden ser más útiles para hacer detección temprana de alteraciones del crecimiento fetal en cada población.

Abstract

Objective: To describe the estimated fetal weight (EFW) according to Hadlock’s formula and the fetal biometric variables: biparietal diameter (BPD), head circumference (HC), abdominal circumference (AC), femoral length (FL) in a sample of pregnant women from a local population and to compare their measurements with the international curves and centiles published by INTERGROWTH-21st and Lagos et al., in Chile. Methods: Data from ultrasound reports of single pregnancies between weeks 16 and 39 of pregnant women in Bogotá-Colombia were obtained. Gestational age (GA) was confirmed by crown-rump length (CRL). Parameters BPD, HC, AC, FL were evaluated and EFW was calculated according to Hadlock's formula. Descriptive analysis of each biometric parameter was made, and the mean, median and standard deviation per week, were calculated. Then, these data were compared with the curves and centiles of the INTERGROWTH-2st project and Latin American curves, using Student's t-test and Wilcoxon signed-rank test. Results: A total of 1133 sonographic reports of 664 fetuses were analyzed, and the data were presented in graphs with the representation of 3rd, 50th and 97th centiles of the INTERGROWTH-21 project and the 10th, 50th, and 90th percentiles of Lagos et al. The AC and FL were the parameters that showed more statistically significant differences with international curves. Conclusions: The reference charts of the INTERGROWTH-21st project and Lagos et al., show differences when fetal growth of this population was compared. The customized or local charts maybe are more useful to reach early detection of alterations of fetal growth in each population.
